SGT. DAVID B. BLEAK TROOP MEDICAL CLINIC (TMC)
The TMC provides health care for Initial Entry Training Soldiers. Sign-in is Monday through Friday, 6:30 to 8:30 a.m. and 12:30 to 2:30 p.m. The clinic is closed from 11:30 a.m. to 12:30 p.m. The clinic also provides health care for ALL Active Duty Soldiers and Marines on Saturdays at 7 a.m. and is closed on Sundays. The clinic is closed on Thanksgiving Day, Christmas Eve, Christmas Day, New Years Eve and New Years Day. On Training Holidays, the clinic will provide limited services. The TMC is located in Building 6039 and staff can be reached at (580) 442-5566.
TROOP MEDICAL CLINIC #1
This TMC provides health care services for Advanced Individual Training Soldiers, Marines, all BOLC, Captain's Career Course, Warrent Officer Students as well as Soldiers in a mobilizing or demobilizing status. Soldiers may walk-in for health problems Monday through Friday, 7 to 8 a.m. and 12:30 to 1:30 p.m. AIT Soldiers and Marines requiring health care on Saturdays will report to the Bleak TMC at 7 a.m. The clinic is closed on Sundays; urgent care will be available at RACH Emergency Room. The clinic is located in Building 2913 and staff can be reached at (580) 442-1310.
EMERGENCY ROOM (ER) SERVICES
The Emergency Room is designed and staffed to provide emergency medical care and is open 24 hours a day, seven days a week. The Emergency Room is not a substitute for Sick Call (page 16) or routine health care needs. Patients are encouraged to call (580) 558 2000, to schedule a same day appointment with their PCM for non- emergent conditions.
MEDICAL SOLDIER READINESS PROCESSING (SRP)
The SRP site provides Pre/Post-Deployment Health Assessment (PDHA), Post-Deployment Health Reassessment (PDHRA) and the annual Periodic Health Assessment (PHA) services. Unit PHA's, PDHA's and PHHRA evaluations require advance scheduling. Walk-ins are seen Monday through Friday, 7:30 to 8 a.m. on a space available basis. The SRP site is located in the basement of Building 4700. To schedule a health assessment or for additional information, call (580) 442-0006.
ONE STATION TRAINING (OST)
OST is the initial medical screening/immunization site for all initial entry Soldiers arriving at Fort Sill. For assistance, call (580) 442-6876.
PHYSICAL EXAMS/ AVIATION HEALTH CARE
Aviation Health Care is conducted at the Physical Exam Clinic along with multiple types of military physicals, i.e., periodic, retirement, separation, etc. Phase 1 and 2 of a physical can be scheduled by calling (580) 558 2228, Monday through Wednesday and Friday, 1 to 3 p.m. Phase 1 information sheets may be picked up at the clinic or viewed on our Internet site. Physicals are for all military components—Active Duty, National Guard and Army Reserves.
